Before Your First Visit

You should contact your insurance carrier to see if they cover the procedure and exactly how much it will cost you & what special requirements they may have.  For, instance, some carriers require an evaluation by a psychologist or a physician-monitored diet for a certain amount of time before approval.

If you have no insurance benefits or do not wish to go through the special requirements or long waiting period, we have arranged for an outstanding finance program with Inamed finance or Capital One Healthcare Finance, which offer convenient & affordable payment options (with flexible

As part of your first visit, we would like to learn more about you and we'll have you fill out a short questionnaire. You can download this form in PDF format here. Please fill out all pages completely & sign the bottom of each & bring this with you to your first visit.

Your First Visit

After taking a few vital signs & calculating your Body Mass Index (BMI), you will be seen by Dr. Bunch or Dr. Erbella. They will go over your questionnaire with you, complete your history & examination and answer all of your questions so that you may make an informed decision.
